TETRA-2142 — 4 calibration custom field trên Appraisal

Rich PA Phase 2 — calibration (chuẩn hoá điểm cross-department). HR điều chỉnh điểm SAU khi appraisal submit (docstatus 1). Lưu điểm hiệu chỉnh tách khỏi final_score gốc do HRMS tính (giữ audit, không amend). allow_on_submit=1 để sửa được trên doc đã submit.

custom_calibrated_score (Float) — điểm sau hiệu chỉnh.
custom_calibration_note (Small Text) — lý do.
custom_calibrated_by (Data, read_only) + custom_calibrated_at (Datetime, read_only) — audit.

Verified: áp 4 field; update custom field trên Appraisal docstatus 1 OK, final_score giữ nguyên (1.333 → 1.333, calibrated=4.5). test_custom_fields 150/150. Bump 0.13.0.
